For example, over the weekend I was on multiple zoom calls with my staff fine tuning the letters in response to the multiple questions from people like senators Rand Paul, Marshall, Burr. I am somewhat surprised that's senator Burr was part of this since he generally is a friendly person to me. We had a very important meeting today by zoom with Mikael Dolsten and others from Pfizer with members of HHS including myself, Rochelle Walensky, David Kessler, Francis Collins, Janet Woodcock, all of the people from ASPR such as Dawn O'Connor. The Pfizer people present the data from the Israeli study which we already knew showed a decrease of efficacy against all infection down to 64%. In addition they showed some of the follow-up data from their phase 3 trial which showed a decrease in efficacy down to 83%. Clearly, Pfizer is intending to suggest boosts particularly for those who are elderly and with underlying conditions . In Israel they are already boosting some of the elderly and those with underlying conditions. There was a big debate in our own country and worldwide about giving a third shot to fully vaccinated people when most of the world is completely unvaccinated. This is a debate that will intensify. I believe we need to go ahead and explore the possibility of needing a boost at the same time as we provide vaccines for people in low and middle income countries. I have been emphasizing in my public appearances that the requirement for a boost does not mean that the vaccines are not highly effective. A boost relates to the durability of the protection not to the effectiveness of the protection. This is a concept that is often missed by the public.
